[Evaluation of the use of virtual reality hypnosis during Port-a-cath® placement in adult oncology patients as an effective alternative to intravenous sedation : a pilot study].

Abstract / Summary

Virtual reality hypnosis (VRH) is a promising tool for managing pain and anxiety during medical procedures. This study evaluated the feasibility of using VRH instead of intravenous sedation (IVS) for implanting a Port-a-cath® (PAC) under local anesthesia in oncology patients. Methods : twenty adults were randomly assigned to receive either IVS (n = 10) or VRH (n = 10) during the procedure. The primary outcome was the successful implantation of the PAC, while secondary outcomes included the need for rescue medication, procedure duration and levels of anxiety. PAC implantation was successful in all patients, with no significant differences in perceived or actual procedure duration between groups. All IVS patients required sedation without rescue medication, while only one patient in the VRH group requested rescue medication. There was no significant difference in procedural anxiety reduction between the two groups and no post-operative complications were reported. VRH is a feasible and safe alternative to IVS for PAC placement under local anesthesia in oncology patients. This innovative tool may become essential in clinical practice by avoiding the potential side effects associated with pharmacological sedation while ensuring a similar patient experience. L’hypnose induite par la réalité virtuelle (HRV) est un outil prometteur pour gérer douleur et anxiété lors de procédures médicales. Cette étude a évalué la faisabilité de l’implantation d’un Port-a-cath® (PAC) sous anesthésie locale associée à la HRV, comparée à la sédation intraveineuse (SIV). Méthodes : vingt patients adultes atteints de cancer ont été répartis au hasard en deux groupes : l’un recevant une SIV (n = 10) et l’autre de la HRV (n = 10) lors de l’implantation. Le principal critère d’évaluation était le succès de l’implantation. Les critères secondaires étaient le besoin d’un protocole de secours, la durée de l’intervention et le niveau d’anxiété. Tous les PAC ont été implantés avec succès. Les patients sous SIV n’ont pas eu besoin du protocole de secours alors qu’un patient dans le groupe HRV y a eu recours. Aucune différence n’a été observée dans la réduction de l’anxiété entre les groupes. Aucune complication postopératoire n’a été rapportée. La HRV est une alternative sûre et efficace à la SIV pour l’implantation de PAC. Cet outil innovant est prometteur pour limiter les effets secondaires de la sédation pharmacologique en assurant au patient un confort similaire.
